Case Management

General Description:

Case Managers/Service Coordinators assist children and adults with a disability in accessing services so that person can live and work in the community.

 

Who is Eligible?

We provide a variety of services from simple information and referral to long term supports.  If you have a question about accessing help for yourself or a loved one with a disability contact us.

 

Once a request for services is received, a case manager/service coordinator will meet with you to assess your needs and devise a plan to meet those needs.  Your case manager/service coordinator will work with you through the entire process to ensure your needs are being met.

 

Services Provided:

Some of the services we provide include, but are not limited to, the following:

 

  • Information and Referral: answering questions about services available in the community & referral to agencies that could meet your needs.
  • Option Counseling: assisting you in knowing what services are available to meet your needs and choosing a service that best meets your needs.
  • Caregiver Support: answering questions and concerns regarding services & supports to assist you with your loved one or client.
  • Abuse Prevention & Intervention: assisting in identifying and accessing supports to prevent abusive situations.
  • Transition Planning: assisting in planning and setting up supports when transitioning from current level of support – includes but not limited to transition from jail, hospitalization, homeless, school graduation, residential placement.
  • Service Coordination/Case Management: on-going support by a social worker to set up services and funding for services to meet long term care needs (residential services, medications, vocational services…).
